Welcome to Spring Creek Elementary!

Since we are a part of Hamilton County Schools, all students looking to attend Spring Creek must be zoned for our school.

Listed below are documents that are required for admission at Spring Creek. We will also complete a records request for the student's previous school as well as get an emergency contact card filled out.

Documents Needed for Registration

(1) 2 proofs of residency                                                         
Must be dated within the last 30 days                       
Examples include:                                             
   Mortage, lease, or deed                
                 Electric, water, gas, or cable bill                
                                                                 Agency or Court Documentation with address (IRS, Disability)            
                              Insurance (medical, life, home, auto) statement
                                                      Current bank statement or employment check stub with date

     (2) Birth Certificate                                                                         
 
 (3) Shot record with physical                                                    
                         This documentation will need to be on the Tennessee form if coming
from another state                                             
                                                     This can be done at any TN Health Department or TN doctor

 (4) Court Papers for Special Cases                                           
                            If you have a special circumstance regarding custody, please provide
                                                      the school with a copy of court documents signed by a magistrate
